Terms of Service

These Terms of Service ("Terms") govern your use of the UtahRX website (the "Site") and the concierge medical, wellness, and aesthetic services ("Services") provided by UtahRX and Elsie Carpenter, PA ("UtahRX," "we," "us," or "our"). By accessing the Site or requesting Services, you agree to these Terms. If you do not agree, please do not use the Site or request Services.

1. Services

UtahRX provides concierge, at-home medical, wellness, and aesthetic treatments including IV injections, Botox, dermal fillers, peptide therapy, regenerative medicine, and family concierge medical care. All Services are performed by licensed medical professionals. Specific treatments are offered only after an individualized consultation and, where clinically indicated, a medical evaluation. We reserve the right to decline or modify any treatment that is not clinically appropriate.

2. No Medical Advice

Information on the Site is for general educational purposes only and is not a substitute for professional medical advice, diagnosis, or treatment. Always seek the advice of a qualified health provider with any questions you may have regarding a medical condition. Never disregard professional medical advice or delay seeking it because of something you have read on this Site.

3. Appointments, Cancellations & Payment

Appointments are scheduled based on availability within our service area (Utah and Salt Lake counties). You agree to provide accurate contact information so we can confirm and coordinate visits. Unless otherwise stated at booking, cancellations made less than 24 hours before a scheduled appointment may be subject to a cancellation fee. Payment terms for each Service are communicated at the time of booking and are due as agreed. Prices are subject to change without notice.

4. SMS / Text Message Communications (A2P 10DLC Consent)

By providing your telephone number through the consultation form or otherwise, and where you have affirmatively opted in, you consent to receive recurring and non-recurring SMS and MMS text messages from UtahRX at the number you provided. Messages may include appointment confirmations, reminders, scheduling updates, follow-up care instructions, and occasional service-related notifications.

Message and data rates may apply. Message frequency varies based on your interactions with us. You can cancel the SMS service at any time by replying STOP to any message. We will confirm your opt-out request. After opting out, you may still receive non-marketing transactional messages (for example, appointment confirmations you explicitly requested). We do not knowingly send marketing SMS messages to wireless numbers without express prior consent. Carriers are not liable for delayed or undelivered messages. We will not share your mobile information with third parties for marketing purposes without your consent.

5. Acceptable Use

You agree not to misuse the Site, including by attempting to access areas not intended for you, interfering with the Site's security, or submitting false or misleading information. You are responsible for the accuracy of the information you provide to us, including contact and health information.

6. Intellectual Property

All content on the Site, including text, graphics, logos, and images, is the property of UtahRX or its licensors and is protected by intellectual property laws. You may not reproduce, distribute, or create derivative works without our prior written permission.

7. Disclaimer of Warranties

The Site and Services are provided "as is" and "as available" without warranties of any kind, whether express or implied, including implied warranties of merchantability or fitness for a particular purpose. We do not guarantee that the Site will be uninterrupted or error-free.

8. Limitation of Liability

To the maximum extent permitted by law, UtahRX and its providers shall not be liable for any indirect, incidental, special, consequential, or punitive damages, or any loss of profits or data, arising out of your use of the Site or Services.

9. Governing Law

These Terms are governed by the laws of the State of Utah, without regard to its conflict of law principles. Any disputes shall be resolved in the courts located in Utah.

10. Changes to These Terms

We may update these Terms from time to time. The "Effective Date" above reflects the most recent revision. Continued use of the Site or Services after changes constitutes acceptance of the updated Terms.
